Our duties
We are required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, to notify you after a breach of unsecured PHI, and to follow the terms of the notice currently in effect.
How we may use and disclose PHI
Treatment: sharing information with providers, pharmacies and laboratories involved in your care. Payment: billing and collecting payment for services. Health care operations: quality review, training, compliance and running our services. We may also use or disclose PHI as required by law, for public health activities, to health oversight agencies, in response to court orders, to prevent a serious threat to health or safety, and for other purposes permitted by HIPAA.
Uses that require your written authorization
Marketing, sale of PHI, and most uses of psychotherapy notes require your written authorization. You may revoke an authorization in writing at any time, except where we have already acted on it.
Your rights
You have the right to: get a copy of your health records; ask us to correct your records; request confidential communications; ask us to limit what we use or share; get a list of those with whom we have shared your information; get a paper copy of this notice; and choose someone to act for you.
Complaints
If you believe your privacy rights have been violated, you may contact us using the details below, or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to this notice
We may change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available on our website.
